Understanding Your Private Health Insurance Options in Spain

Private health insurance is popular with Americans in Spain because it offers quick appointments, access to private hospitals and clinics, and English-speaking support. But "private health insurance" covers several different plan types, and the one that is right for you depends on how you like to use healthcare and what you want to pay.

This page focuses on the product itself: the main plan types, what they include, and how copayments, cover levels and underwriting work. If you want the wider context — using cover day to day as a resident, moving to Spain, or a visa application — we link to the right page as we go.

Compare the Types

Private Health Insurance Plan Types Compared

Spanish private health insurance comes in a few main forms. Most Americans choose between these, and the right fit depends on your budget and how you expect to use care.

Full Cover (Network Plans)

Outpatient and inpatient care within the insurer's network — GPs, specialists, tests, hospital treatment and surgery. The most common choice for residents.

Basic / Outpatient Cover

More affordable modular plans covering GPs, specialists and tests, but not hospitalisation. Lower cost, narrower cover.

No-Copayment Plans (Sin Copago)

No per-visit charge and predictable costs, at a higher premium. Often expected for visa applications.

Copayment Plans (Con Copago)

A lower premium with a small fixed fee each time you use a service — useful if you expect to use care less often.

Reimbursement / Premium (Reembolso)

The most flexible tier: use providers outside the network and claim back a share of the cost, usually at a higher premium.

Family & Combined Plans

One policy covering couples, children and dependants, with each person assessed on their own age and health.

There is rarely a single “best” plan. The right choice balances the premium, how often you expect to use care, whether you want hospital cover as well as outpatient, and whether a visa requires a specific type. We compare these trade-offs against how you actually expect to use healthcare in Spain, rather than just the headline price.

Typical Cover

What Private Health Insurance in Spain Can Include

Exact cover and limits vary by plan and insurer, but most Spanish private plans include the essentials below, with extras available on higher tiers.

GP & Specialist Care

Appointments with family doctors and specialists in the network.

Diagnostic Tests

Scans, blood tests and investigations as covered by your plan.

Hospital Treatment & Surgery

Private hospital cover for planned and emergency care, and surgery, on full plans.

Maternity

Maternity benefits on some plans, often after a waiting period.

Dental & Extras

Basic dental and other extras included or added on certain plans.

Family Cover

Couples, children and dependants on one policy.

The Key Choice

Copayment vs No-Copayment Plans Explained

One of the biggest decisions is whether to take a copayment or no-copayment plan. Neither is simply "better" — it comes down to how often you expect to use healthcare and whether you prefer a lower premium or fully predictable costs.

No-Copayment (Sin Copago)

You pay a higher premium but nothing per visit, so your yearly cost is predictable. Popular with frequent users, families, and applicants who need visa-suitable cover.

Copayment (Con Copago)

You pay a lower premium plus a small fixed fee each time you use a service. Often works out cheaper if you use care only occasionally.

Copayment amounts and which services they apply to vary by insurer and plan. We can compare the real yearly cost of each for how you actually expect to use care.

Underwriting

Pre-Existing Conditions and Underwriting

Whichever plan type you choose, insurers underwrite each application individually, and how they treat pre-existing conditions affects your cover and price. Always declare known conditions and current medication honestly when you apply.

Depending on the insurer, a condition may be covered, subject to specific terms, or excluded, and some plans apply waiting periods before certain treatments (such as maternity or surgery) are covered. Acceptance and terms always rest with the insurer, so we cannot promise a particular outcome — but we can help you present your situation clearly and compare insurers more likely to consider it.

What to know about applying

  • Declare all known conditions and current medication
  • Insurers underwrite each application individually
  • A condition may be covered, excluded or have special terms
  • Some plans apply waiting periods for certain treatments
  • Acceptance and terms rest with the insurer
  • Ask us before assuming a condition rules you out
Which Do You Need?

Visa-Suitable Cover vs Everyday Private Cover

Not all private cover is the same in the eyes of a Spanish consulate. If you need a policy for a visa, it usually has to meet specific conditions — typically full private cover with no copayments and a certificate for the application — whereas everyday private cover gives you more freedom to choose the plan type that suits you.

Coming From the US

How Spanish Private Cover Differs From US Insurance

Spanish private health insurance is built around care in Spain and generally costs far less than US cover, but it works differently. Most plans use an insurer network of doctors and hospitals, premiums are based mainly on age and cover level, and there is no direct equivalent to a US deductible — instead you have the copayment or no-copayment structure described above.

Importantly, your US insurance and Medicare generally will not cover you in Spain, so a Spanish policy is usually your day-to-day cover here. For what does and does not carry over from the US, see our Medicare and health insurance in Spain for Americans page.

Households & Changes

Families, Couples and Switching Plans

Most plan types can cover a whole household. Couples, spouses, children and dependants are usually quoted together on one arrangement, with each person assessed on their own age and health, which keeps things simple.

Your choice of plan is not permanent either — you can compare and switch at renewal as your needs change. If you already live in Spain and want help using, renewing or switching cover, see health insurance for American expats in Spain. If you are retired or approaching retirement, health insurance for American retirees in Spain covers older-applicant options in more detail.
