Spanish public healthcare access depends on the access pathway:

  • Employment Social Security contributions: NLV holders are typically not working, so this route doesn’t open via NLV.
  • S1 form (UK retirees): State Pensioners can register S1 for public access with UK paying. Available to UK retirees who’ve reached State Pension age.
  • Convenio especial: pay-in arrangement for public access. Available in most autonomous regions. Typical cost €60–€160 per month depending on age.

Pre-existing conditions

Common at retirement age. Spanish-licensed insurers underwrite through disclosure. Typical conditions handled: controlled hypertension, controlled cholesterol, well-controlled diabetes, joint replacement history (older), prior cancer treatment with 5+ years cancer-free, stable cardiac conditions, stable mental health.

Serious recent conditions are more likely to be declined, postponed or excluded: active cancer treatment, recent major cardiac events, uncontrolled diabetes with complications, recent stroke, recent hospitalisation, complex mental health, multiple serious conditions. Payment evidence — why annual upfront is safest

For NLV visa applications, the safest approach is normally to include proof that the first 12 months of the health insurance policy have been paid upfront. Some policies may allow split payment, depending on the insurer and product, but visa applicants should be careful: a certificate supported only by a first monthly payment can create avoidable questions or rejection risk at consulate stage.

Payment options for visa-style policies: Spanish or EU IBAN, bank transfer, or credit/debit card. Annual upfront payment by credit/debit card is the cleanest route for visa files. Two-payment instalments may be available where a Spanish bank account is in place.

Renewals and ongoing cover

After 5 years, permanent residency opens up. The strict NLV-specific compliance markers no longer formally apply. Options:
