Madrid's international community — Americans, British, Irish, Latin Americans, corporate expats, digital nomads — all need insurance arranged in English, by specialists who understand living in a major Spanish city. We are here seven days a week.

Madrid is Spain's capital and its largest city — a metropolis of more than three million people in the urban core and over six million in the wider metropolitan area. For expats, Madrid offers a compelling combination: excellent infrastructure, a world-class cultural life, outstanding food and nightlife, and an increasingly sophisticated international business environment. The tech sector has grown enormously in recent years, drawing American, British, and European professionals to the city alongside the longer-established communities of Latin American expats and the traditional corporate expat population in finance and consulting. Madrid has also become the most popular destination for Spain's Digital Nomad Visa, attracting location-independent workers from across the English-speaking world.
Living in Madrid as an expat is a genuinely urban experience. Unlike the Costa Blanca or Costa del Sol, where expat life is often centred on communities of similar nationalities, Madrid integrates you into a fast-paced city with high rents, complex bureaucracy, and — for those unfamiliar with Spanish institutions — occasional frustrations navigating health, insurance, and administrative systems in a second language. That is exactly where 247 Expat Insurance steps in: we handle the insurance complexity so you can focus on the city.

Madrid's expat community is concentrated in certain neighbourhoods: Salamanca, Chamberí, and Retiro attract wealthy international families; Malasaña, Chueca, and Lavapiés draw younger expats and digital nomads; the suburbs of Pozuelo de Alarcón, Las Rozas, and La Moraleja are popular with corporate expat families who value larger homes, international schools, and green space. Each of these settings comes with slightly different insurance needs.
Your Cover Explained
Madrid's higher cost of living, large private hospital network, complex traffic environment, and diverse expat population create specific insurance requirements. Here is what you need to know.
Madrid has an extensive and well-regarded private healthcare sector. The city's size means there is genuine competition between private hospital groups, and a good health insurance policy with a broad Madrid network gives you access to consultants, diagnostics, and procedures at facilities that operate to an excellent standard. For those on non-lucrative visas or digital nomad visas, private health insurance is a legal requirement — the policy must meet specific criteria set by the Spanish consulate or the immigration authorities, including minimum coverage levels and an absence of limitations for pre-existing conditions in some cases.
Choosing the right health insurance policy in Madrid is not simply a matter of finding the cheapest premium. The hospital network is critical: some insurers offer broad access to multiple hospital groups across the Madrid metropolitan area, while others are restricted to a narrower panel. If you live in Pozuelo, a policy with the best coverage in Salamanca is of limited help. We compare network breadth carefully and match your policy to where you live, work, and are likely to need treatment.
For family expats with children at international schools such as those in Las Rozas, Alcobendas, or central Madrid, a family health insurance policy that covers all household members — including dental and paediatric care — is an important consideration. We arrange family policies from leading Spanish and international insurers and can include maternity cover where needed.
Driving in Madrid is a different proposition to driving in a coastal resort town. The city has one of Europe's more complex urban road networks, a well-used but often confusing metro system that does not prevent the surface roads from being extremely busy, and an ambitious low-emission zone — Madrid Central, extended under the Madrid 360 scheme — that restricts access to much of the city centre for older, more polluting vehicles. Fines for entering the restricted zone in an ineligible vehicle can be significant and accumulate rapidly.
Parking in central Madrid is a persistent challenge. Residential parking permits (SER zones) apply across much of the inner city, and street parking for non-residents in central areas is extremely limited. Car theft and break-in theft from vehicles are a concern in any major city, and we recommend comprehensive cover for Madrid residents rather than just the minimum third-party liability. We can add legal protection, breakdown cover, and a courtesy vehicle as part of your policy.
For those who have recently relocated from the UK and still hold a UK-registered vehicle, we can advise on the 30-day rule and help you plan the transition to a Spanish-registered vehicle and a Spanish insurance policy. Those who use a vehicle primarily for occasional weekend trips out of the city may have different needs to daily commuters, and we tailor cover accordingly.
Madrid's property market is a tale of two situations for expats. Many newcomers rent, at least initially — and the Madrid rental market is expensive, with central apartments commanding very high monthly rents. For renters, the building itself is the landlord's responsibility to insure, but your contents are entirely your own responsibility. A contents insurance policy covers furniture, electronics, clothing, jewellery, and personal belongings against theft, fire, water damage, and accidental damage. Some policies extend to tenant liability — protecting you if you accidentally damage the property you are renting.
For expats who buy property in Madrid — whether an apartment in Salamanca or a detached villa in La Moraleja — buildings insurance is essential. The comunidad de propietarios (owners' community) in an apartment block typically insures the building structure and communal areas, but this does not remove the need for your own policy covering the interior of your apartment, your contents, and your personal liability. We advise all apartment buyers in Madrid to check what the comunidad policy covers before assuming they are adequately protected.
Madrid is the most popular city in Spain for holders of the Digital Nomad Visa (DNV), also known as the Startups Law visa. The DNV allows non-EU nationals who work remotely for non-Spanish employers or clients to live legally in Spain for an extended period. Private health insurance is a mandatory requirement of the visa application — the policy must cover you fully in Spain without significant exclusions or co-payments, and you will need a certificate of insurance to present to the Spanish consulate. We are experienced in arranging DNV-compliant policies and in providing the paperwork you need to complete your application.

It depends on your status. Non-lucrative visa holders and digital nomad visa holders are required by Spanish law to hold private health insurance. Corporate expats contributing to social security can access the public system, but many prefer private cover for faster specialist access and English-language consultations. Madrid has an excellent range of private hospitals, and a good policy ensures you can access them efficiently.
Madrid Central (extended as Madrid 360) is a low-emission zone covering much of the city centre. Access is restricted for older, more polluting vehicles, and fines for non-compliance can be significant. While the zone does not directly affect your insurance premium, we advise on vehicle eligibility and ensure your comprehensive car insurance is appropriate for Madrid city driving.
The Spanish Digital Nomad Visa requires private health insurance that covers you fully in Spain. The policy must meet specific criteria. We are experienced in arranging DNV-compliant health insurance policies and can provide a certificate of insurance in the format required by the Spanish consulate or the relevant immigration authorities.
If you are renting, the building itself is the landlord's responsibility, but your contents are not. A contents insurance policy protects your possessions against theft, fire, water damage, and accidental damage. Some policies also include tenant liability cover in case you accidentally damage the property. We can arrange contents-only cover for renters throughout Madrid.
Yes. Family health insurance policies cover all named dependants, including children. Many international school families in Madrid hold a family policy covering routine GP appointments, specialist consultations, dental treatment, and emergency care for all family members. We help families compare policy options across different insurer networks to find the right fit.
Theft of and from vehicles is a concern in any major city. A comprehensive car insurance policy in Madrid will include cover for vehicle theft, attempted theft damage, and in many cases contents stolen from the vehicle up to a specified limit. If you park regularly on the street, we recommend discussing this when arranging your policy to ensure the terms are appropriate.
Whether you have just arrived or have been in Madrid for years, we can review your cover and make sure you have the right policies in place. Seven days a week, no Spanish required.
