Once you’re Spanish resident, your home-country travel insurance arrangements typically need to be replaced. Most travel insurers require residency in the country of purchase — UK travel insurance bought from the UK as a Spanish resident may not be valid unless the policy specifically accepts Spanish residents. Spanish residents have several options: Spanish-purchased travel insurance, broker-arranged international cover, or annual multi-trip policies suited to expat travel patterns.

EHIC / TSE for EU travel

Spanish residents registered with the public Spanish healthcare system can apply for the European Health Insurance Card (EHIC, called Tarjeta Sanitaria Europea / TSE in Spanish). This provides emergency public-system access in EU/EEA/Switzerland during temporary stays. Important: EHIC/TSE doesn’t replace travel insurance — it doesn’t cover repatriation, lost luggage, trip cancellation, private treatment or non-emergency care. Travel insurance is still strongly recommended for all EU trips.

GHIC for UK residents (and former UK residents)

The UK Global Health Insurance Card (GHIC) is for UK residents travelling to the EU. It doesn’t apply to UK nationals who are Spanish residents. UK residents visiting Spain have GHIC; Spanish residents travelling back to the UK don’t have an equivalent reciprocal card.

S1 holders

UK state pensioners with an S1 living in Spain may continue to have UK NHS treatment access during temporary UK visits, but this is not a substitute for travel insurance.

Single-trip vs annual multi-trip

Single-trip: for individual journeys. Premium based on destination, duration, age, trip cost.

Annual multi-trip: for travellers making multiple trips per year. Typically includes unlimited trips within Europe up to a per-trip day limit (often 31 days), with selected longer trips and optional worldwide upgrades. Many expats find annual multi-trip the most cost-effective.

Worldwide / USA cover considerations

USA travel attracts significantly higher premium loadings due to high US medical costs. Worldwide-including-USA cover is typically a separate tier with substantially higher premium. Cruise cover sometimes requires worldwide-including-USA or sea-cruise add-on.

Cruise cover

Cruises have specific cover requirements: missed departure, port skipped due to weather, medical care onboard ship, helicopter evacuation. Verify cruise-cover is included if relevant. Some annual multi-trip policies exclude cruise unless cruise add-on is selected.

Adventure sport and ski cover
