Non-Lucrative Visa Health Insurance with Pre-Existing Conditions
Applying for Spain’s Non-Lucrative Visa with a health condition to declare? Insurers assess every application individually, so cover is never guaranteed — but many conditions can still be insured. We help NLV applicants of any nationality declare their conditions honestly and look for visa-suitable cover with no copayments and full cover in Spain.
Arranging Cover When You Have a Condition to Declare
If you are looking for non-lucrative visa health insurance with pre-existing conditions, this page explains honestly how it works. It is for NLV applicants of any nationality with a condition to declare — diabetes, a heart condition, recovery after surgery, ongoing treatment, or a cancer history.
No broker or insurer can promise in advance that a condition will be covered. Insurers assess each application individually: some conditions are accepted on normal terms, some with an exclusion, a waiting period or a higher premium, and occasionally an application is declined. Being open about your health from the start gives you the best chance of arranging suitable cover.
At 247 Expat Insurance we help expats arrange Spanish private health insurance. We are an insurance intermediary, not an immigration adviser: we cannot decide your visa, but we can help you present your medical history to insurers and understand the terms you are offered.
Non-Lucrative Visa Health Insurance With Pre-Existing Conditions: How Insurers Decide
The insurer reviews the medical information you declare and decides on what terms — if any — it can offer cover. This is underwriting, and the outcome depends on the condition, how well it is controlled, your treatment and your age.
There are broadly four outcomes, and it helps to know them before you start:
Accepted on Standard Terms
The condition is insured normally, with no special terms on your policy.
Accepted With an Exclusion
Cover is offered, but claims relating to the declared condition are excluded. The rest of your cover is unaffected.
Waiting Period or Loaded Premium
Cover is offered subject to a waiting period, or with an additional premium reflecting the extra risk.
Cover Declined
Sometimes an insurer cannot offer cover for a condition or applicant. Another may still consider you.
Outcomes differ between insurers, so it is worth comparing more than one option. We cannot promise acceptance, but we can help you approach insurers more likely to consider your situation.
The Medical Questionnaire: What to Declare
Most applications involving a health condition include a medical questionnaire. Complete it fully and truthfully. Non-disclosure — even accidental — can lead to claims being refused or a policy cancelled later, which could affect your residency.
It helps to have the following ready:
- All current and past medical conditions
- Any medication you take, with doses
- Recent or planned surgery and dates
- Ongoing treatment or specialist care
- Key dates, such as diagnosis or last treatment
- Any medical reports your doctor can provide
Common Conditions Applicants Ask About
Every history is different, and the notes below are general guidance rather than a decision. Each condition is assessed individually, and your terms depend on your circumstances.
Diabetes
Well-controlled diabetes is often insurable, sometimes with an exclusion or waiting period; type, control and medication matter.
Heart Conditions
Treated or stable heart conditions may be considered, often with an exclusion; recent events are looked at more cautiously.
After Cancer
A cancer history is assessed on how long you have been clear, and your follow-up; some insurers consider cover after a period in remission.
After Surgery
Once you have recovered from surgery, cover is often available; ongoing complications or planned procedures are assessed separately.
Ongoing Treatment
If you are still in treatment or take regular medication, insurers want current details before deciding terms.
Mental Health
Conditions such as anxiety or depression are assessed individually, considering current treatment and stability.
What a Certificate With Exclusions Means for Your Visa
If your policy is accepted with an exclusion, does it still work for the Non-Lucrative Visa? It may still be usable, depending on the policy, the certificate and the office reviewing the application. The visa requires a qualifying private health policy — full cover in Spain, no copayments, no waiting periods for the required cover, from an insurer authorised to operate in Spain. A condition-specific exclusion does not automatically mean the policy is unsuitable, but it should be reviewed carefully before submission.
The certificate you present confirms the cover and its validity. It is the qualifying policy, not the absence of an exclusion, that the requirement focuses on. Requirements can vary between consulates and extranjería offices, and visa decisions are always made by the Spanish authorities. We can explain what your certificate shows.
Declare Honestly and Protect Your Existing Cover
Two habits protect you more than anything else here:
Please keep these in mind
- Declare every condition truthfully — non-disclosure can void a policy just when you need it most.
- Do not cancel existing health insurance until your new policy is accepted and in force.
- Read any exclusions or waiting periods carefully so you know what is and is not covered.
- Keep medical evidence to hand, as insurers may ask for reports.
- No one can promise acceptance in advance; insurers assess each application individually.
If a previous application was declined, or came back with unexpected terms, tell us what happened and we will help you look again.
