🇨🇦 For Canadians Relocating to Spain

Health Insurance for Canadians Moving to Spain

Leaving Toronto, Vancouver, Calgary or Montreal for a new life on the Mediterranean? The one thing that will not travel with you is your provincial health card. Health insurance for Canadians moving to Spain fills that gap from the day you land — private doctors, specialists and hospitals, arranged from Canada before you fly.

Your Provincial Health Card Stops at the Border

Canadians are used to healthcare that simply happens: you show a card, you are seen, and the billing sorts itself out. The card in your wallet is a provincial entitlement, not a passport benefit — it works because you live in Ontario, British Columbia, Alberta or Quebec, not because you are Canadian.

Settle in Spain and that link breaks. No reciprocal arrangement lets a Canadian use a provincial plan at a Spanish health centre, and arriving does not put you into the Spanish public system automatically — that generally follows from working and paying social security contributions, or from certain registered-resident routes.

Private cover bridges the gap, which is why health insurance for Canadians moving to Spain is usually organised before the flight rather than after arrival.

Leaving Canada

What Happens to OHIP, MSP, AHCIP and RAMQ When You Leave

Provincial health insurance is built around being ordinarily resident in the province. Each province sets its own limit on how long you can be away before entitlement lapses, and those limits are not generous when the absence is a permanent move rather than a winter down south.

Tell your province you are emigrating and coverage ends. Say nothing and it will still typically lapse once you pass the absence limit — which matters if you ever return and have to re-establish residency before cover restarts. Because the detail varies, check your position with your provincial health ministry before you go. It is a short call, and it tells you the date your Spanish cover must be live by.

Canadian cover that will not carry you through a move to Spain

  • Provincial plans (OHIP, MSP, AHCIP, RAMQ and the rest)
  • Out-of-country provincial benefits (emergency-only, Canadian rates)
  • Snowbird and travel policies
  • Credit card travel insurance
  • Employer group benefits you leave behind
  • Retiree extended health plans
  • Student out-of-province plans
  • Emergency-only international policies

Private Health Insurance as a New Resident in Spain

A Spanish private health policy is not the travel insurance you buy for holidays. It is built for people who live here: routine appointments, a GP you can go back to, specialists, diagnostics, maternity, surgery and private hospitals.

The pricing surprises most Canadians: private medical cover in Spain generally costs a fraction of comparable cover in North America.

Plans come in two shapes. No-copayment policies fold everything into the premium, so you pay nothing further when you use the cover — the shape most visa and residency routes expect. Copayment policies run a lower premium with a small charge per visit, which suits lighter users. We work with leading Spanish insurers and will talk you through which fits.

Everyday Care

GP appointments, specialists, diagnostics and tests, usually without long waits.

Hospital Treatment

Access to private hospitals and clinics across Spain.

Language Support

English-speaking doctors are widely available in the main expat regions.

Family Cover

Partners, children and dependants quoted together on one policy.

Choosing Well

How to Choose a Plan Before You Land

Choosing from six time zones away is easier than it sounds. A handful of questions settle most of it, and none require you to have found an apartment yet.

  • Where in Spain you are heading — hospital networks vary by region
  • Who needs cover, and everyone’s ages
  • Whether paperwork must satisfy a visa or residency application
  • Copayment or no-copayment, based on expected use
  • Existing medical conditions — insurers assess each application individually
  • Waiting periods, and whether your route needs them removed
  • Your arrival date, so cover starts on time
  • Whether you want dental or extras bolted on
On timing. Give yourself a few weeks. Policies can be set up quickly, but comparing properly is far easier with runway than in the fortnight before a transatlantic flight.

The Visa Question

If Your Move Also Needs a Visa

Canadians can visit the Schengen area visa-free for short stays, but living in Spain needs a long-stay route. Where that applies, the policy must meet specific conditions rather than simply exist — consulates and extranjería offices typically ask for full cover in Spain with no copayments, no waiting periods and a certificate for the file.

That is a subject in itself, so we cover it separately. Start with the guide, then your route.

Non-Lucrative Visa

For Canadians living on savings or pension income, without working here. See NLV health insurance for Canadians.

Digital Nomad Visa

For Canadians working remotely for non-Spanish companies or clients. See DNV health insurance for Canadians.

Most Canadians start a few weeks to a few months before departure. That leaves time to compare plans without rushing, match the start date to your arrival, and have paperwork ready if a visa needs it.
