For a healthy resident who sees a doctor twice a year, a copay plan may well be cheaper over a year. But your visa file is not a budgeting exercise. It is a document reviewed against a standard, and against that standard the copayment is not a saving. It is a gap.
Why Visa Applications Expect No Copay Health Insurance for a Spanish Visa
Spain grants you the right to live there on condition that you will not burden its public healthcare, so your cover is measured against the public system — and a resident using that system is not billed at the door for an appointment.

The Copayment Traps That Delay Applications
Most copayment problems are not carelessness. A copayment can hide in the small print of a policy that looks fine on the surface.
Where copayments hide
- The quote defaulted to copay — comparison tools show the cheapest version first, and that is nearly always the copayment one.
- Copays on part of the policy only — some plans drop them for GP visits but keep them on specialists or diagnostics. Partial removal still counts.
- The word never appears — the charge may be called a “franquicia”, a per-act fee or a contribution. If you pay at the point of care, it counts.
- A certificate that stays silent — if it does not state the absence of copayments, the reviewer cannot assume it.

Waiting periods, or períodos de carencia, are the months an insurer might require before certain treatment becomes available — surgery, childbirth and some specialist procedures are the usual examples. Cover that would not respond in your first weeks in Spain is, to a reviewer, the same problem in a new shape. The two conditions travel together, so a visa policy should have both removed at once.
The Certificate That Proves the Copayments Are Gone
Applicants underestimate this part: buying the right policy and proving you bought it are two separate tasks. Your file is not judged on what your cover does — it is judged on what your certificate says.
