The insurer reviews the medical information you declare and decides on what terms — if any — it can offer cover. This is underwriting, and the outcome depends on the condition, how well it is controlled, your treatment and your age.

There are broadly four outcomes, and it helps to know them before you start:

Accepted on Standard Terms

The condition is insured normally, with no special terms on your policy.

Accepted With an Exclusion

Cover is offered, but claims relating to the declared condition are excluded. The rest of your cover is unaffected.

Waiting Period or Loaded Premium

Cover is offered subject to a waiting period, or with an additional premium reflecting the extra risk.

Cover Declined

Sometimes an insurer cannot offer cover for a condition or applicant. Another may still consider you.

Most applications involving a health condition include a medical questionnaire. Complete it fully and truthfully. Non-disclosure — even accidental — can lead to claims being refused or a policy cancelled later, which could affect your residency.

It helps to have the following ready:

  • Any medication you take, with doses
  • Recent or planned surgery and dates
  • Ongoing treatment or specialist care
  • Key dates, such as diagnosis or last treatment
  • Any medical reports your doctor can provide

What a Certificate With Exclusions Means for Your Visa

If your policy is accepted with an exclusion, does it still work for the Non-Lucrative Visa? It may still be usable, depending on the policy, the certificate and the office reviewing the application. The visa requires a qualifying private health policy — full cover in Spain, no copayments, no waiting periods for the required cover, from an insurer authorised to operate in Spain. A condition-specific exclusion does not automatically mean the policy is unsuitable, but it should be reviewed carefully before submission.

The certificate you present confirms the cover and its validity. It is the qualifying policy, not the absence of an exclusion, that the requirement focuses on.
