Provincial plans such as OHIP (Ontario), RAMQ (Quebec), MSP (British Columbia) and AHCIP (Alberta) cover residents receiving care in Canada. They do not pay for treatment in Spain and do not produce the certificate a Spanish consulate needs for the NLV.
Extended residence abroad can affect provincial eligibility
Provincial rules vary, but extended residence outside Canada can affect your eligibility for provincial healthcare. Snowbird Cover vs Spanish Residency Cover
Many Canadians are familiar with snowbird insurance for spending part of the year outside Canada. That is not the same as health insurance for Spanish residency. Snowbird insurance is normally designed around temporary travel, return-to-Canada assumptions, emergency treatment and limited trip durations. The document the consulate actually reads is the insurance certificate, so it matters that it says the right things.
