Health Insurance for British Citizens Moving to Spain
Planning your move from the UK to Spain? Since Brexit, British citizens are third-country nationals, and private health insurance now plays a much bigger part in relocating. 247 Expat Insurance helps you compare private health cover with clear English-speaking support — whether you are moving as an individual, a couple, a family or a retiree.
Do British Citizens Need Private Health Insurance in Spain?
Usually yes — but which route applies to you comes down to one question: who pays for your healthcare.
If you draw a UK State Pension or another qualifying exportable benefit, the UK may pay for your Spanish healthcare through an S1 form registered with the INSS. If you work or are self-employed in Spain and pay Spanish social security, you are entitled to state healthcare on the same basis as a Spanish worker, and so are your dependants. If neither applies, you will normally need private health insurance. GOV.UK states it plainly: if you have lived in Spain for less than one year and cannot get healthcare cover another way, you will need to buy private health insurance.
A GHIC is not a residence solution. It is for temporary stays, including a period of study, not for everyday healthcare once you live in Spain. And proof of healthcare cover is needed twice over — when you apply for a visa, and again before you can register as a resident.
Drawing a UK State Pension
An S1 may cover you and your dependants through the Spanish state system. You cannot also join the Convenio Especial.
Working in Spain
Spanish social security contributions give you state cover. Many British residents still add private cover for speed and English-speaking care.
Neither of those
Private health insurance from a Spanish-authorised insurer is the normal route, and the one a visa office expects to see.
Private Health Cover for Your Move from the UK to Spain
Choosing health insurance for British citizens moving to Spain is one of the first practical decisions of the relocation. Before 2021, UK nationals could move under EU free movement and slot into the Spanish system as Europeans. Now, as third-country nationals, many British movers need a long-stay visa, and private health cover may be part of the route depending on their circumstances.
The right policy depends on your age, where you are settling, who is moving with you and whether the cover also has to satisfy a visa or residency route. Some people simply want reliable private healthcare once they arrive; others need a policy that a consulate will accept as part of the application.
Depending on your plans, you may also find our Spanish visa health insurance for UK citizens, Non-Lucrative Visa health insurance for British citizens or Digital Nomad Visa health insurance for British citizens pages helpful as you plan.
Why British Movers Choose Private Health Insurance in Spain
Spain has a well-regarded healthcare system, but private cover gives new arrivals more control while they settle in. For many British movers, it is not only about being ready for the visa. It is about making everyday healthcare simpler in the early months, when you are still finding your feet, registering locally and getting to know your new area.
- Cover in place from the day you arrive
- Faster access to private specialists and appointments
- Access to private clinics and hospitals
- English-speaking support choosing and using cover
- Reassurance for couples and families relocating together
- No-copayment options available
- Visa-suitable options if your route needs them
- Support from a Spain-based team who know the move
Can You Rely on a GHIC or the NHS After Moving to Spain?
Many British movers assume their existing UK arrangements will carry over. In practice, a GHIC is designed for temporary or emergency treatment during short visits, not for everyday healthcare once you live in Spain as a resident. The NHS is a UK service and does not follow you abroad.
UK travel insurance and short-stay policies are also built for holidays and trips, not for settling permanently. They usually will not provide the ongoing private cover you want as a resident, and they are not designed as proof of cover for a Spanish visa or residency application.
UK cover that is usually not enough once you move
- A GHIC (temporary and emergency use only)
- The NHS once you are resident in Spain
- UK travel insurance
- Credit card travel cover
- Short-stay or holiday policies
- Emergency-only international cover
- Annual multi-trip travel plans
- Old EHIC assumptions from before Brexit

Everyday Private Cover vs Visa-Suitable Cover
There is an important difference between everyday private health insurance and cover chosen specifically for a visa.
Everyday private health insurance is for British movers who want private healthcare access once they live in Spain. Visa-suitable cover is chosen when a Spanish visa or residency route requires a specific type of policy, often with no copayments. If your move involves a visa, we will point you to the right route rather than guess. 247 Expat Insurance is an insurance intermediary and not an immigration adviser, and residency decisions are always made by the Spanish authorities.
Everyday Private Cover

GHIC, S1 and Private Insurance: Which One Actually Applies
These are not alternatives you pick between on price. They are different mechanisms, and usually only one of them is open to you at a given time.
GHIC or EHIC
For temporary stays: short visits, a period of study, or a posting by a UK employer. A Student GHIC covers medically necessary state treatment for the duration of a course. It is not cover for living in Spain as a resident, and it is not proof of cover for a long-stay visa.
S1 form
UK-funded state healthcare for residents of Spain who draw a UK State Pension or another qualifying exportable benefit, and in some cases frontier workers. Requested by telephone from NHS Overseas Healthcare Services, part of the NHS Business Services Authority, then registered with the INSS. Once registered, you and your dependants get state healthcare on the same basis as a Spanish citizen, plus a UK-issued GHIC for travel.
Convenio Especial
Paying monthly into the Spanish public scheme directly. It requires at least one year of registration on the padrón, so it is not available in your first year. Prescriptions are not subsidised on it: you pay the full cost, as well as non-urgent transport, orthopaedic supplies, prostheses and dietary supplies.
Private health insurance
The route for everyone else, and the only one of the four that works from your first day in Spain. It is also what a consulate or visa office expects to see as proof of cover, and the only one you can have in place before you leave the UK.
Two consequences worth knowing. If the UK pays for your healthcare through an S1, you cannot join the Convenio Especial, and you cannot register for healthcare as a permanent resident instead. And if you hold an S1 as the dependant of a State Pensioner, that cover is cancelled once you begin claiming your own UK State Pension: a new S1 is issued and has to be registered with the INSS again, or your cover lapses.
If you are at or near pension age, read our guide to the S1 form for UK pensioners in Spain alongside this page.
The Route You Apply On Changes What the Policy Must Say
British applicants are third-country nationals, so the route matters. The documentary standard is not the same across all four, and guidance written for one route is regularly misapplied to another.
Non-Lucrative Visa
The regulation's own wording requires only that an applicant hold health insurance. Conditions about copayments and waiting periods are documentary expectations set by the office handling your file rather than words in the regulation — but offices do commonly ask for them.
Digital Nomad Visa
Here the stricter standard is written down. The processing unit's own instruction sheet does not accept travel insurance, policies based only on reimbursement of medical expenses, or policies with copayments or waiting periods.
Student route
The interpretive criteria call for comprehensive cover from an insurer authorised in Spain, free of copayments, reimbursements and waiting periods. A working, registered student may need no private policy for this requirement at all.
Joining family
Each person on the application normally needs their own certificate, priced on their own age. A spouse and children are not covered by the main applicant's certificate by default.
247 Expat Insurance arranges the insurance for these routes. We do not prepare or submit the visa application itself.
When to Arrange Cover, and What the Certificate Has to Show
Arrange cover from the UK before you travel. Proof of healthcare cover is needed when you apply for the visa and again before you can register as a resident, so the policy has to exist before either step rather than after you land.
- A certificate in the applicant's name, matching the passport exactly
- Cover running for the full period of the authorisation you are requesting
- An insurer authorised to operate in Spain, named on the certificate
- The policy number and the start date stated on the document
- One certificate for each person named on the application
- Where the route expects it, confirmation of no copayments and no waiting periods
Allow time for the certificate itself. It is issued once the policy is set up, and appointment dates rarely move to suit a document that has not arrived.
Moving Before Pension Age, and Moving After It
Before pension age. There is no S1 route open to you, and no Convenio Especial in your first year either. Private cover is effectively the only option, and it is also what the visa office will expect to see. This is the group for whom insurance matters most and is most often underestimated.
At or after pension age. An S1 may make the Spanish state system available to you and your dependants. Many British residents still keep private cover alongside it, for appointment speed, choice of specialist and English-speaking consultations — but the requirement picture is different, and it is worth not buying as though it were the same.
Crossing that line while already living in Spain. This is the case people miss. Beginning to draw your UK State Pension changes your position, and if you were covered as a dependant on someone else's S1 it ends that cover. Review your arrangements in the months before you start claiming, not afterwards.
Age and premium. Age is the single largest factor in what a Spanish policy costs, and medical history weighs more heavily the later you buy. Arranging cover before a move rather than after it is usually the cheaper order of events.
Common Mistakes British Movers Make
Mistakes that cost time, money or a refusal
- Assuming a GHIC covers life in Spain, when it covers temporary stays
- Expecting the NHS to follow them; it is residency-based, and moving abroad permanently can end free entitlement
- Offering UK travel insurance or credit-card cover as visa proof
- Planning on the Convenio Especial in year one, when it needs a year on the padrón first
- Holding an S1 and also applying for the Convenio Especial or permanent-resident healthcare — you cannot do both
- Not registering the new S1 issued after starting to claim a UK State Pension
- Buying a policy shorter than the authorisation applied for
- One certificate for a couple or a family, when each person needs their own
- Leaving cover until after arrival, when proof is needed at the visa stage
- Reading a digital nomad or student requirement across to the non-lucrative route, or the reverse
