Before Brexit, many British citizens had different healthcare routes in Spain. Now, new UK applicants are treated as third-country nationals, and for the Non-Lucrative Visa private health insurance is normally required.
The policy needs to meet the visa requirements — not just be a normal expat policy. Why British Applicants Need Private Health Insurance After Brexit
As non-EU applicants, British citizens applying for the NLV are normally expected to hold private cover. In practice this means:
- British citizens are now non-EU applicants for the NLV
- The Spanish authorities expect private health insurance
- The policy should normally be no-copayment
- It should provide full cover in Spain
- It should be from an insurer authorised in Spain
- The applicant needs a certificate for the consulate
Can You Use the NHS, GHIC or an S1 for the NLV?
NHS
The NHS does not act as private health insurance for a Spanish NLV application, as it is a UK-based service.
GHIC
A GHIC is useful for temporary or emergency access, but it is not a substitute for comprehensive private health insurance for the NLV.
S1
Some British pensioners may have S1 entitlement, but it must be checked carefully for the visa route and your circumstances.
In short, the NHS is a UK-based service and should not be treated as private Spanish health insurance for residence in Spain, and a GHIC or EHIC only covers necessary care during temporary visits, not residence. Private UK health insurance such as Bupa, AXA Health or Vitality is built around UK providers and will not usually produce the Spanish-format certificate or meet the NLV requirements, and travel insurance is short-term visitor cover that is not accepted either.

UK cover that usually is not suitable for the NLV
- The NHS and a GHIC or EHIC
- Private UK health insurance not issued by a Spanish-authorised insurer
- Travel or trip insurance
- General policies with copayments or that do not cover you fully in Spain
The S1 Route for UK State Pensioners
The S1 is the genuine alternative to buying private insurance, but it is not for everyone. If you receive the UK State Pension, or certain other exportable UK benefits, you may be entitled to an S1 form, which lets you register for Spanish state healthcare with the cost covered by the UK. Where an S1 applies and is accepted for your application route, it may satisfy the healthcare requirement without a private policy.
The important point is who it does not cover. If you are under UK state pension age, or you are not otherwise S1-eligible, you will normally still need a private Spanish-authorised policy for the Non-Lucrative Visa — and many British applicants, especially early retirees, fall into this group. What Your NLV Health Insurance Must Include
Requirements can vary by consulate, but an NLV-suitable policy generally needs to:

Issued by an insurer authorised to operate in Spain.

Consulates increasingly expect no-copayment (sin copago) cover for the NLV, because a copayment can be read as the applicant carrying part of the cost rather than being fully insured. Choosing a no-copayment NLV policy from the start avoids a query on your file.
Your Insurance Certificate — What It Must Say and Proof of Payment
The document your consulate or BLS centre reads is the insurance certificate, so the wording matters. It should confirm a full private health policy with a Spanish-authorised insurer, valid in Spain, with no copayments and no waiting period on the required cover, and it should name each insured person.
NLV policies are usually paid annually in advance, and the certificate should reflect cover that is paid and in force — so keep your proof of payment with it. When to Buy — Timing Before Your Consulate or BLS Appointment
UK applicants apply for the NLV through the Spanish Consulate for their region (London, Manchester or Edinburgh) and its appointed visa centre, usually BLS, and appointment slots can be limited. Plan backwards from your appointment date and have the policy and certificate ready before you attend, because they form part of the documents you submit.
