Why US Health Insurance Doesn't Work in Spain

This surprises many Americans. Medicare, the federal health insurance programme for people over 65, does not provide coverage outside the United States — with very limited exceptions. ACA marketplace plans (the plans purchased through healthcare.gov or state exchanges) are likewise domestic plans tied to US providers and networks. Private US health insurance from major US carriers typically does not include Spanish coverage at the level required for a Spanish visa.

For your Spanish residency application — whether via the Non-Lucrative Visa (NLV) or the Digital Nomad Visa (DNV) — you must present private health insurance that meets Spain's specific requirements: full coverage in Spain, no copayments (a concept very different from US healthcare, where copays are the norm), and issued by an insurer registered with Spain's insurance regulator, the DGSFP.

Key point: Spanish NLV health insurance must have no copayments. This means you pay nothing at the point of care. This is a hard requirement at the consulate level and is fundamentally different from the US model of deductibles, copays, and out-of-pocket maximums.

The Non-Lucrative Visa (NLV) for Americans

The NLV is the most popular route for Americans moving to Spain who are not planning to work for a Spanish employer or run a Spanish business. It suits retirees, people with investment income, landlords, and anyone who can demonstrate sufficient passive income to support themselves without working in Spain.

The application is made at the Spanish consulate in your US state of residence. The main consulates processing NLV applications are in New York (covering the north-east), Miami (the south-east and Latin America), Los Angeles (the west coast), Chicago (the Midwest), and Houston (Texas and surrounding states). Each consulate has its own specific requirements, document checklists, and appointment processes, and our team is familiar with the variations between them.

The Digital Nomad Visa (DNV) for Americans

If you work remotely for a non-Spanish employer — whether as a freelancer or an employee — the DNV may be the right route. It was introduced by Spain in 2023 and has proved popular with American tech workers, consultants, writers, and creatives who want the structure of a legal residency. The DNV also requires private health insurance meeting the same no-copayment standard as the NLV.

Car Insurance for Americans in Spain

Car insurance is compulsory in Spain, and there is an important deadline on the licence side. A non-EU licence is valid for driving in Spain for a maximum of six months, counted from the date you acquire normal residence (residencia normal) here. Spain does not currently have a general bilateral licence exchange agreement with the United States, so a US state licence cannot normally be exchanged for a Spanish one, and that applies whichever state issued it. Most American residents who want to keep driving must instead obtain a Spanish licence from scratch through the DGT (Dirección General de Tráfico), passing both the theory test and the practical driving test. For car categories the DGT theory test can be sat in English, French or German as well as in Spanish and the co-official regional languages; the practical test is conducted in Spanish. During those first six months your US licence is generally valid for driving in Spain, provided it is accompanied by an official translation or by an International Driving Permit, which serves as that translation and must always be carried with the licence itself. Car insurance is arranged as normal in the meantime.
