But Brexit changed the landscape significantly, and British nationals now face a different set of rules, requirements, and challenges when it comes to insurance in Spain.

Health Insurance for British Expats in Spain

Health insurance is typically the first and most pressing concern for British expats moving to Spain. Unlike EU nationals who can access Spain's public health system through reciprocal agreements, British nationals post-Brexit are treated as third-country nationals for most healthcare purposes.

If you are applying for the Non-Lucrative Visa (NLV) — the most common route for British retirees and individuals with passive income — Spanish law requires you to hold private health insurance with full coverage in Spain and no copayments. The policy must be issued by a company registered with Spain's insurance regulator, the DGSFP (Dirección General de Seguros y Fondos de Pensiones). Policies purchased in the UK, including international health insurance plans, are typically not accepted at Spanish consulates unless they meet these specific requirements.

The Digital Nomad Visa (DNV), which has become increasingly popular with British remote workers and freelancers, also requires private health insurance as part of the application. Given that most tech-savvy professionals moving to Spain's major cities under this visa are relatively young and healthy, comprehensive private cover is both affordable and essential.

Important: Your GHIC (Global Health Insurance Card) is not a substitute for private health insurance as a Spanish resident. The GHIC covers emergency treatment during temporary stays in EU countries — once you are a resident, it no longer applies, and it has never been accepted as proof of health cover for visa purposes.

If you are of pension age and receive a UK State Pension, you may be eligible for an S1 form, which entitles you to healthcare through Spain's public system, with the cost recharged to the UK government. Qualifying for an S1 is genuinely valuable, but it does not provide access to private hospitals or specialist care without waiting. Many British pensioner expats hold both an S1 and a private health insurance policy to get the best of both systems.

Car Insurance for British Expats in Spain

Car insurance is compulsory in Spain, as in the UK. However, the process of insuring your vehicle as a British expat involves a few additional considerations that catch many people off guard.

Firstly, your UK no-claims bonus. Spanish insurers are not legally obliged to accept UK no-claims history, but many will take it into account — typically in the form of a letter from your previous UK insurer confirming the number of claim-free years. Providing this documentation when you apply for Spanish car insurance can make a meaningful difference to your premium.

Secondly, and more significantly, there is the question of your driving licence. Since Brexit, UK driving licences are not automatically recognised for driving in Spain as a resident. The UK and Spain have agreed arrangements for the exchange of licences, but the process can take time, and there have been periods of uncertainty. We recommend that British expats check the current DGT (Dirección General de Tráfico) guidance and begin the exchange process promptly after registering as a resident. Your car insurance policy remains valid during this period, but completing the exchange avoids any risk of complications.

Home insurance in Spain typically covers the building structure (known as continente) and the contents (contenido) separately, and it is important to insure both to adequate replacement value.

For British expats who own a property in Spain but still have a home in the UK — or who split their time between the two — there are specific considerations. A Spanish property left unoccupied for extended periods may require a specialist policy that covers unoccupancy, as standard policies often include clauses that limit cover if the property is empty for more than 30 or 60 days. Do I need private health insurance as a British expat in Spain?

It entitles the holder to healthcare in Spain via the Spanish public health system, charged back to the UK. If you are of pension age and receive a UK State Pension, you may qualify for an S1. However, the S1 does not provide private healthcare access, so many British pensioner expats still take out private health insurance for faster access and comfort.
