The most important thing to understand is that Australian healthcare — public or private — is built to cover you in Australia. The NLV requires cover that works in Spain, issued by a Spanish-authorised insurer. That mismatch is why almost no Australian applicant can use their existing arrangements.
Australian Medicare stops at the border
Medicare is a domestic system: it does not pay for treatment you receive in Spain, and it is not recognised by Spanish consulates as NLV health cover. Medicare is not designed to act as private health insurance for residence in Spain, so it should not be relied on for the NLV application.
There is no reciprocal healthcare agreement between Australia and Spain
Australia has Reciprocal Health Care Agreements (RHCAs) with a number of countries — including the UK, New Zealand and Ireland — but Spain is not one of them.
