Plenty of pages will tell you the rules. This one is about the purchase: which policy to put in your file, how the certificate reaches you, who else goes on it, and what happens twelve months later when it renews.

Private health insurance for residency in Spain is a specific product. It is contracted in Spain, with a Spanish insurer, on Spanish terms — and the version sold to residents is deliberately different from the version sold to visitors. The visitor product assumes you fly home for anything serious. The resident product assumes Spain is home, which is exactly the assumption your application depends on.

Products people buy by mistake

  • Expat or international medical plans — often excellent cover, but frequently written with reimbursement models or copayments that do not fit a residency file.
  • Travel and trip policies — priced for weeks, not years, and built around repatriation rather than living here.
  • The cheap Spanish plan with a small copayment — a fine product for someone already resident, but the copayment is the problem.
  • A home-country employer or state scheme — a GHIC or EHIC covers temporary stays only, and Medicare does not cover healthcare in Spain at all.

Your Insurance Certificate and How You Get It

The policy is what protects you; the certificate is what proves it. Consulates and extranjería offices typically ask for a certificate issued by the insurer — on insurer letterhead, naming each insured person, confirming the cover is full, that no copayments apply, that no waiting periods apply, and stating the start date.

A receipt is not a certificate. Nor is a policy booklet, a bank statement or a screenshot of a payment. Applications get bounced on this distinction more often than on the cover itself, which is why we obtain the certificate for you rather than leaving you to request it.

Residency is usually a household affair, and the insurance follows suit. Everyone in the application goes on one policy, with one premium collected on one date and one renewal to remember — but each person receives their own certificate, because each person is proving their own cover.

Premiums are calculated per person and rise with age, so a couple in their sixties pays considerably more than a family with young children. Children are typically the cheapest people on any policy. If a family member joins you later — a partner following on, a baby born in Spain — they can normally be added mid-term and their certificate issued straight away, without disturbing anyone else’s cover.

Declare any medical history honestly for every person on the policy. Insurers assess each application individually, and something declared and accepted is worth far more than something hidden that surfaces at claim time.

Year Two and Beyond

Renewals, and Switching Insurer Without Breaking Cover

Buying private health insurance for residency in Spain is not a one-off transaction. Your policy renews annually and your premium goes up as you age, so at some point you will look at what else is available — and that is where people get into trouble.

The rule is simple: overlap, never gap. Continuous cover is the thing your residency renewal cares about, so a new policy must start on or before the day the old one ends. Cancel first and shop afterwards and you create a documented break, which is a great deal harder to explain than it is to avoid.

When we move a client between insurers we set the new start date, obtain the new certificate, confirm it says what it should, and only then cancel the old policy within its notice period. Spanish policies generally require written notice before the renewal date, so a switch is worth starting a month or two ahead rather than the week before.

Buying and Keeping Residency Cover: Your Questions

Look for a policy from an insurer authorised in Spain that gives full private cover throughout Spain, charges no copayments, applies no waiting periods to the cover you are relying on, and does not lapse if you spend time outside the country. A policy built for residency will tick all of those boxes on the schedule, not just in the sales blurb.
Once your policy is contracted and the first payment is set up, the certificate can usually be issued within a working day or two, and often the same day if your paperwork is complete. Tell us your appointment or submission deadline when you request a quote so we can work back from it.
Yes. Most residency applications are made as a family unit, and leading Spanish insurers will place everyone on one policy with one payment and one renewal date. Each person still needs their own certificate showing their own name and date of birth, and we arrange those together.
The policy renews automatically each year unless you cancel it, and the premium is recalculated at your new age. Because a lapse in cover can cause problems at renewal of your residency card, we flag your renewal date in advance and reissue a certificate whenever you need a current one.
You can, but the two policies must overlap rather than leave a gap. We arrange the new cover to start on or before the day the old one ends, obtain the new certificate first, and only then help you cancel the previous policy within its notice period.
Insurers assess each application individually, and a declared condition may be accepted, excluded or loaded depending on what it is and its history. Always declare it honestly, because an undeclared condition can invalidate a claim and leave you without the cover your residency relies on.
